It’s simple, robust and gives you the choice of two methodologies and custom categories. Plus it does the fiddly work for you by auto-calculating values and shading the results to reflect the risk severity.
Plus. I’ll send you my 5-day mini risk assessment course to help you plan your assessment and set yourself up for success.

The template is fully customizable and comes with a complete set of instructions so you can get to work right away.
I’ve used this type of risk assessment template hundreds of times for everything from a quick and dirty table-top review to a weeks-long country assessment.
If you’re looking for a simple risk assessment template, this is what you need.
But wait. Before you start filling in a spreadsheet, you need to make sure you’re set up for success.
Here’s how
Setting Your Risk Assessment up for Success
Hopefully you’ve already planned out your assessment and are ready to complete the risk assessment template. But if not, let’s roll things back for a moment to make sure it’s going to give you what you need. Take a second to review the three steps below and, if you aren’t ready to move on, use the resources listed to help you out.
- Step 1 – make sure you’ve clearly defined the scope and purpose of the assessment. Otherwise, you’ll have a complete assessment but won’t be able to put those results into context. Read more on this and get a planning guide here.
- Step 2 – check the gradings and metrics make sense to you and that these will make sense to others in your organization. There’s a description of gradings and metrics here.
- Step 3 – think about how you will turn the results you get from the risk assessment template into recommendations. Maybe you will use the ordered list of risks to prioritize where to allocate resources. Perhaps you need to also design mitigation measures to address the higher risks. Whatever your role (which you clarified in step 1), make sure you know how your risk assessment results will support that.
